Chronic Condition Care Plans by Phone

From 1 November 2026, Medicare allows GPs to prepare or review a Chronic Condition Management Plan over the phone. This means some patients with long-term health conditions may not need to attend the clinic in person for these appointments.

What is a Chronic Condition Management Plan?

A Chronic Condition Management Plan is a plan created by your GP to help manage a long-term health condition. It can include your health goals, treatment, and referrals to other health professionals.

What has changed?

Before 1 November 2026, a GP could only prepare or review these plans face-to-face or by video call. From 1 November 2026, they can also do this by telephone and claim Medicare benefits for the service.

Who can use this service?

Patients with a chronic or long-term health condition may be able to have their care plan prepared or reviewed by phone if their GP decides this is appropriate.

Important things to know
  • The phone care plan items do not need a 12-month face-to-face relationship if conducted by your eligible telehealth practitioner.
  • If you are registered with MyMedicare, your care plan must be completed by your registered GP or practice.
  • Having a care plan prepared by phone does not change your access to allied health services. Eligible patients can still access up to five allied health visits each calendar year under their care plan.
  • Phone appointments can be helpful for people who cannot easily attend the clinic or use video technology.
What should patients do?

If you think a phone appointment might work better for you:

  1. Contact your GP clinic.
  2. Ask if your Chronic Condition Management Plan can be prepared or reviewed by phone.
  3. If you are registered with MyMedicare, let the clinic know.

This new option may make it easier for people with chronic illnesses to access care, especially if travelling to appointments or using video calls is difficult.

Do I need to join MyMedicare?

No. You do not have to be registered with MyMedicare to have a Chronic Condition Management Plan prepared or reviewed by phone.

However, if you are already registered with MyMedicare, your care plan must be done by your registered GP or clinic.

If you are unsure whether you are registered with MyMedicare, ask your GP clinic.
