Community Packages (ComPacks)

Description: Short-term, non-clinical case management and home support for NSW public hospital patients who are medically stable but at risk of readmission without additional support.
Community Transport

Description: Community transport is a service for eligible seniors, pensioners, those with accessibility needs or those who have limited access to public transport. The service is especially designed to help you get to medical appointments, participate in social activities, do your shopping and maintain independent living at home.
Isolated Patients Travel and Accommodation Assistance Scheme (IPTAAS)
Location: New South Wales
Description: The Isolated Patients Travel and Accommodation Assistance Scheme (IPTAAS) provides financial support to eligible NSW residents, including Lord Howe Island, to help cover travel and accommodation for specialised health treatment not available locally. To qualify, patients must be enrolled in Medicare, not receive similar support elsewhere, and travel at least 100 km one way (or 200 km per week) to the nearest approved health service.
My Care Partners program

NSW Systems Coordinator Program
Location: New South Wales
Description: The NSW Systems Coordinator Program provides case management for people under 65 with complex care needs who are ineligible for NDIS or Commonwealth-funded aged care. Systems Coordinators across NSW help individuals access supports and services across health, community, disability, and housing sectors.
Planned Care for Better Health
Location: New South Wales
Description: The Planned Care for Better Health (PCBH) program supports people to self-manage their health conditions by providing education, coordinating care, and helping navigate healthcare and community services. The PCBH team works with a broad range of services, including allied health, mental health, social work, Centrelink, and cardiopulmonary services, to ensure individuals receive the medical and social support they need.
Sir Roden and Lady Cutler
Location: New South Wales
Description:Sir Roden and Lady Cutler Patient Transport Service provides free transport for people who need to travel to or from a health facility but do not require an emergency ambulance, including wheelchair-accessible options. Services most areas of metro Sydney.
Safe and Supported at Home (SASH)
Location: New South Wales
Description: The SASH (Safe and Supported at Home) package provides short-term support for people with functional impairments who are applying for the NDIS or are ineligible, helping them safely transition from hospital or access community health services.
