Health@Home Pathways Program
Location: Victoria
Description: The Health@Home Pathways Program supports adults (18+) in Victoria living with chronic health conditions. Nurses and health coaches help participants understand their conditions, connect with local services, build skills to manage their health, stay well at home, and prevent hospital visits.
Home and Community Care for Younger People (HACC PYP)
Location: Victoria
Description: The Home and Community Care Program for Younger People (HACC PYP) supports Victorians (0–65 years) and Aboriginal and Torres Strait Islander people (0–50 years) who need help with daily tasks such as personal care, meals, domestic chores, property upkeep, community access, or transport. It assists people with disability, chronic illness, or short-term health issues who are not eligible for NDIS, helping them live as independently as possible at home. Services are delivered through local councils, health services, Aboriginal and multicultural organisations, and other community providers, usually with a small copayment. Disability Liaison Officer Program
Location: Victoria
Description:
- help with barriers to booking appointments
- arranging reasonable adjustments
- mobility/physical support
- communication assistance
- psychosocial support.
Multiple and Complex Needs Initiative
Location: Victoria
Description:
- provide information and advice on how to support a person in a complex service situation
- explore other support options, including referrals for extra assessments or services
- help with case direction and decision making to deliver holistic care
- help to improve coordination of supports and services alongside National Disability Insurance Scheme (NDIS) funded support coordinators.

Right Care Better Health Program
Location: Victoria
Description: Living with chronic health conditions can be complex. Our nurses and health coaches work with you to build confidence in managing your health and staying well at home.
Notes: You will need a referral from your GP. GP needs to be from the participating practice.
Statewide Equipment Service (SWEP)
Location: Victoria
Description: The Victorian Aids & Equipment Program (A&EP) provides subsidised Assistive Technology (AT) items and home modifications to enhance independence in the home, facilitate community participation and support families and carers in their role.
