Community Transport Program

Home Assist and Secure

Description: Home Assist Secure (HAS) provides advice, referrals, and financial support to help eligible Queensland homeowners and renters, aged 60+ or any age with a disability, carry out critical home maintenance, repairs, minor modifications, and safety upgrades. Services may include handyperson or licensed trades work, yard maintenance, smoke alarm installation, and security improvements, with subsidies covering labour costs (materials usually paid by the client).
Hospital in the Home (HITH)

Description:Hospital in the Home (HITH) provides hospital-level care at home for patients recently discharged or requiring hospital-level treatment, offering daily monitoring, treatment, and support from a multidisciplinary healthcare team until they can safely transition back to GP care.

Queensland Community Support Scheme (QCSS)

Description:The Queensland Community Support Scheme (QCSS) provides low-intensity support to help people under 65 (under 50 for Aboriginal and Torres Strait Islander people) who are not eligible for NDIS to live independently, reduce social isolation, and participate in their community. QCSS can assist with daily living tasks, personal care, meal preparation, household chores, essential yard maintenance, accessing services, attending recreational activities, and connecting with other supports and services.
Supported Accommodation Providers Association (SAPA)

Description: Supported Accommodation offers affordable, stable housing for people who need extra support in their daily lives. Accommodation is provided in managed facilities with shared amenities, and meals are often included. Eligibility requirements vary by provider — contact providers directly for more information.
Taxi Subsidy Scheme
Location: Queensland
Description: The Taxi Subsidy Scheme helps Queensland residents with permanent or severe disabilities who have difficulty using public transport. It covers 50% of the taxi fare (up to $30 per trip), with no limit on the number of trips. Members must reapply every five years.
